Community Support Team

 

The purpose of our Community Support Team is to improve the quality of life for individuals with mental illness. To attain this goal we use a case management model employing the principles of Psychosocial Rehabilitation with a client centered focus. Emphasis is placed on maximizing strengths and enhancing self esteem.

Community Support Team Features

  • Services provided by this team are suitable for individuals requiring moderate levels of support (2 hrs max per week)
  • Case management model ie. each client is assigned a primary worker to co-ordinate and link services and provide ongoing support
  • Staff client ratio 1:20
  • Regular appointments will be scheduled with primary workers during daytime hours according to client needs
  • 24 hour access to crisis support and intervention - 7 days per week
  • Supports may be provided in the client's home, in the community, or in the office

What We Offer

  • Individualized Assessment and Goal Planning
  • Assisting clients to cope with and/or overcome symptoms and impairments caused by their mental illness
  • Case management which could include planning, advocacy, linking and co-ordination of appropriate services
  • Crisis Response and Intervention
  • Consultation and co-ordination with other Community Supports
